Upload Supporting Documentation




Newly hired and existing employees can now submit all of their documentation through Box.com. Select the appropriate tab listed below for more information.

Newly Hired


For information about your benefits options, please refer to our New Hire benefits page. Here you can find lots of great information, and also watch our orientation videos.

  • You must submit your Life Insurance Beneficiary Form and your Benefits Acknowledgment Form on or before your date of hire. Click on the “Submit Button” in order to email your completed form to the benefits team.
  • Complete and submit your enrollment form (and your HSA Election Form if applicable) within 30 calendars days of your date of hire. If you miss this deadline, you will be automatically enrolled in CORE coverage.
  • If you plan to enroll eligible dependents on a County plan, you must include all required dependent documentation (ie. Birth Certifcate, Marriage License, etc) with your enrollment form. For details, refer to the Eligibility & Rules section of your Benefits Handbook.
  • Refer to the submission steps tab.
  • While you are eligible for benefits on your date of hire, your coverage will not begin until all required documentation has been received and processed.

Existing Employees


Qualified Events: If you are experiencing a mid-year life event you may qualify to make changes to your existing benefits elections.

  • For Marriage, Birth of a Child, Adoption/Legal Custody/Guardianship, or Divorce submit your request online via myOCPortal within 60 calendars days of your qualified event.
  • For all other qualified events, complete & submit your enrollment form within 60 calendars days of your qualified event.
    • Proof of your life event is required and must be submitted along with your enrollment form.
    • If you plan to enroll new dependents, you must include all required dependent documentation (ie. Birth Certifcate, Marriage License, etc) with your enrollment form. For details, refer to the Eligibility & Rules section of your Benefits Handbook.
    • Refer to the submission steps tab.
    • Approved changes will be effective within 1-2 pay periods.

    Open Enrollment: All benefits eligible employees may make changes to their benefits annually during open enrollment in October.

    • Submit your election changes online via myOCPortal during the annual open enrollment period.
    • If you plan to enroll new dependents, you must submit all required dependent documentation (ie. Birth Certifcate, Marriage License, etc) within the posted deadline. For details, refer to the Eligibility & Rules section of your Benefits Handbook.
    • Refer to the submission steps tab.
    • Approved changes will be effective beginning on January 1st.

    Life Insurance Beneficiaries: Employees may change their life insurance beneficiaries anytime.

    • Submit your changes easily online via myOCPortal, self-service.
    • Alternatively, you may submit a paper Life Insurance Beneficiary Form. Click on the “Submit Button” in order to email your completed form to the benefits team.
    • Approved changes will be effective within 1-2 pay periods.

    Health Savings Account (HSA): Employees enrolled in the High Deductible Medical plan may change their HSA contributions anytime

    • Submit your completed HSA Election Form
    • Refer to the submission steps tab.
    • Approved changes will be effective within 1-2 pay periods.

Submission Steps


How to submit your benefits documentation:

  1. Applicable forms and supporting documentation must be submitted by the deadlines outlined above.

  2. Individually save each forms and/or supporting document as follows:

    Employee ID + Last Name + Document Type

    123456 LastName Enrollment Form
    123456 LastName Spouse Name
    123456 LastName Child1 Name
    123456 LastName Child2 Name

  3. Scan and upload your forms and supporting documentation to our secure Box.com folder.
  • Select or drag your document(s) into the secure Box.com folder, then click submit. The progress window will display the status of each upload until 100% has been reached.
  • Once completed, a confirmation page will display. Human Resources will reach out if anything additional is needed. Approved changes will be effective as outlined above.
  • The maximum file size is 5 MB, and only the following file types may be used: .pdf (unlocked), .jpg, .jpeg, .gif, .tiff, .bmp, and .txt
